Two cardiology bodies have set out how clinicians should judge whether someone is fit to answer the next call, filling a gap the writing group's vice chair says he spotted after decades of guidance written for competitive sport.

Start with the timing, because it is the most specific empirical thing in the release. Lili Barouch, who directs sports cardiology at Johns Hopkins Medicine and did not work on the document, told STAT that the firefighter event rate climbs from the moment the station alarm sounds, with cardiac events appearing before crews reach the building [11]. The guidance puts the danger window at the alarm and running through suppression [10]. Benjamin Levine, vice chair of the writing group and director of the Institute for Exercise and Environmental Medicine at Texas Health Presbyterian Dallas [7], described the physiology plainly: no warm-up, no weeks of preparation, called out and expected to perform at a high level immediately, with what he called a risk of death that is not insignificant [8]. That timing is a real distribution, and it says the exposure is not only heat and hauling gear. It also cannot rank causes. A sudden sympathetic surge and pre-existing coronary disease would both put events near the alarm, and knowing when something happens does not identify what did it.
Two dates are worth putting together. Some of the authors have been writing recommendations for competitive athletes since that work began in the 1980s [14], the tactical athlete was first recognized as a category in the early 2000s [5], and STAT reported the release on Sept. 3, 2026 [18]. So: roughly forty years of competitive-sport cardiology, and a little over twenty years between naming this population and giving it a document of its own [19][20]. By Levine's telling the gap closed through geography, since the AHA's national headquarters sit next door to his institute, so he walked in and made the case to its leadership [15].
The thing this doesn't tell you is whether evaluating people this way changes the number of deaths. The recommendations cover how to assess tactical athletes who have cardiovascular disease or are at risk of developing it, in light of what their duties demand [22], which is consensus about process. No event rate for the four million people in scope appears in the account, and sudden cardiac arrest and death are described as low-rate apart from a rise during recruit training [9][3]. In a group that large with an outcome that uncommon, most of what a standard produces is reassurance and referral rather than a save, and every referral is somebody off the truck until the testing is finished.
The most usable content is the least exotic. Alongside heavy equipment, extreme heat and cold, deep water and altitude, the guidance names the ordinary risk factors: high blood pressure, high cholesterol, obesity, type 2 diabetes, smoking [12]. And because these careers run long, the authors treat these workers as closer to masters athletes, defined as 40 and older, whose cardiovascular risk rises with age much as their nonathletic peers' does [13]. The occupation is what makes the population unusual. The treatable risk looks like the risk in any other clinic.
Levine's account of the stakes is what makes the fitness-for-duty call hard in both directions: a colleague who goes down inside a fire leaves the other firefighter alone in there, and affects the people they went in to rescue [17]. Clearing someone who should be benched and benching someone who should be cleared each put a third party inside the decision. Sports cardiologists told STAT they welcome the framework [16]. A written standard is the right thing to want here; the outcome data behind it is the next thing to want.

That round is unusually well-populated — the writing group's vice chair on the record, plus two sports cardiologists who did not draft the thing — but the guidance in JACC and Circulation is described, not examined, and the 4 million estimate and the top-cause-of-death ranking are relayed from the authors rather than checked against any external tally.
Published and applauded, not yet practised
What exists is a document in two journals and warm words from cardiologists who read it. No fire service, EMS agency, military health command or NASA flight-medicine office appears anywhere in this reporting saying it will screen differently on Monday, and for guidance whose entire point is fitness-for-duty decisions, the employer is the adopter that matters.
Urgency a step ahead of the event rate
The framing — most important professionals you've never heard of, four million of them, cardiovascular disease as the top duty-related killer — sits a little forward of the numbers in the same piece, which concede that absolute rates of sudden cardiac arrest are low and, via Churchill, that these jobs aggravate existing disease rather than cause it. The gap is small and self-correcting, because STAT prints the caveats rather than burying them.
A champion who tells you he lobbied
The vice chair says outright that he walked into AHA headquarters next door and talked the leadership into commissioning this, and he arrives with four decades of investment in competitive-athlete guidance plus consulting roles with NASA and North Texas teams. That is a clear stake in the category existing — and it is disclosed in the open, with the two outside voices STAT quotes carrying no drafting interest of their own.
Specific reporting, one vantage point
The particulars hold up: named clinicians, named institutions, a dated release, quotes that qualify each other. What holds confidence down is structural rather than sloppy — a single outlet, no reading of the recommendations themselves, and nothing at all on whether the employers who make fitness-for-duty calls intend to follow them.
