Infrared imaging shows gland dysfunction in 85 percent of frequent eyelash extension users
Tetiana Zhmud's team surveyed 345 women aged 16 to 28, then imaged the eyelid oil glands of those who had worn extensions more than ten times. Signs of dysfunction showed up in 85 percent of them.

What happened

Why it matters
- constraint Nobody age-matched without extensions was imaged, so the 85 percent cannot be converted into a risk attributable to extensions. A clinician cannot quote it to a patient as her chance of gland damage.
- decision A woman choosing between stopping extensions and adding a twice-daily gel-and-brush routine is deciding on an ophthalmologist's judgement. The study reports no outcome difference between users who cleaned their lids and users who did not.

- contradiction Ni Dhubhghaill made two points the clinic has to weigh together: the mites can be normal residents of the eye, yet heavy loads are unusual at this age. A positive finding only changes management alongside symptoms and inflammation.
Meibomian glands sit inside the eyelid and secrete the oily layer that slows tear evaporation; when they clog or underproduce, the eye turns dry and irritated [7]. Zhmud's team looked at them with meibography, which uses infrared light to see the glands through the lid [5]. In the women who had had extensions more than ten times, they reported signs of dysfunction in 85 percent [6].
The report does not say how many women were in that subgroup, but the survey numbers bound it. A quarter of 345 is 86, and almost a quarter of the surveyed women had tried extensions [2]. So every extension-related figure here rests on fewer than about 86 people, and the ten-or-more group is a slice of that [1]. The discomfort figure implies a denominator too: 47.6 percent [4] is what 40 out of 84 comes to [4].
The work was presented at the annual congress of the European Society of Cataract and Refractive Surgeons [1]. The authors say plainly that the study was small and had no comparison group of women who never wore extensions [15]. Eighty-five percent describes the women who were imaged, and they were all extension users. Sorcha Ni Dhubhghaill of UZ Brussel, who chairs the ESCRS Cornea Committee and was not involved, said Demodex folliculorum mites are common but are not usually found in large numbers in younger women [17]. Her impression comes from clinical practice. The team's own prescription is a larger study comparing people with and without extensions, and testing whether mite numbers, how often extensions are applied, and eye symptoms are connected [16].
Under the microscope, 24 of 25 regular users carried D. folliculorum [8], which is 96 percent [2]. Eighteen of the 25, 72 percent, had signs associated with infestation: itchy or inflamed lids, redness, swelling, lashes sticking together, and flaky skin at the lash roots [9][3]. Presence by itself is not disease, since the mites can be part of the normal microbial population around the eye [19]. "But it is important to detect them if they are causing symptoms, signs of inflammation and meibomian gland dysfunction," Ni Dhubhghaill said [20]. Detection is harder in this group, she noted, because the adhesive is applied close to the roots of the natural lashes and can make the waxy collarettes that mark the mites harder to spot [18].
The hygiene advice goes beyond what the study tested. The researchers say the findings are not a reason to give up extensions, and point instead to keeping the lids clean and watching for symptoms [10]. Zhmud, of the National Pirogov Memorial Medical University in Vinnytsia, recommends a preservative-free, hypoallergenic gel formulated for therapeutic eyelid hygiene, used twice a day with an eyelash cleansing brush [11]. Where an infestation is confirmed, she recommends anti-parasitic treatment too [12]. The study did not put that regimen to a test; users who cleaned their lids and users who did not were never compared, so the mitigation claim rests on clinical judgement. In my view the regimen is sensible practice for a lash line under adhesive twice a month, and the study does not tell you how much it helps.
"At the same time, there is growing evidence suggesting that this procedure may create problems on the surface of the eye, including irritation, dry eye symptoms, and meibomian gland dysfunction," Zhmud said in a statement [14].
What to watch
- A full paper from the Vinnytsia group that reports how many women were in the meibography subgroup and how gland loss was scored.
- Whether other centres publish Demodex rates in young women of the same age who have never worn extensions.
- Any ESCRS or lash-industry guidance that turns the eyelid hygiene recommendation into a stated standard of care.
