A keyless detector built to catch sandwich attacks on public DEX data found two in 8,000 swaps. In the pool CoinMarketCap ranked first by 24-hour transactions, three wallets were buying back what they had just sold inside single transactions.
Reality
CoinMarketCap's DEX liquidity endpoint accepts a maker= filter that returns one wallet's whole history on a token. A dev.to run used it to follow 335 removals of $100k or more on an 11-token, 3-chain watchlist.

CounterCall's builder pointed a voice agent at Singapore's immigration enquiries line and watched it talk to a touch-tone menu, because the platform's OpenAPI spec has no field for sending DTMF tones.

Ofcom's photosensitive-epilepsy rule is written in seconds, so the rate you extract frames at ends up inside the specification. The same overlooked parameter broke the SQL detector first and the model that adjudicates its output two days later.
Reality
A modelContext that registers tools correctly can still refuse addEventListener, and the throw lands after the page has painted. The repair is a bounded poll plus a banner that names the mechanism actually carrying the app.

According to a developer's write-up, six contracts satisfy the EXTCODESIZE(owner) > 0 firmness check on an on-chain order book and still pull their depth, each run as a real testnet transaction.
Reality
A Solana MCP server ran for three months with two write tools that built transactions, discarded them, and returned success. Full coverage, all tests passing, nothing on chain.
